Public Health Services: HIV testing/Counseling, Lead Screenings, Communicable Disease Control, Health Education Information, School Health Consultation, and Referrals for Special Needs Individuals.
Family Health Services: WIC – (Women, Infants and Children) Program is a nutritional food program for pregnant women, breast feeding women, infants and children up to 5 years of age. Individuals must meet income guidelines and will be seen by appointment.
Birth Certificates: Birth Certificates for those born in the State of Missouri only. Birth Certificates are available from years 1920 to present.
Death Certificates: Death Certificates are for those in the State of Missouri only. Death Certificates are available from years 1980 to present
Environmental Services: CCHD provides inspections of Food, Lodging, Licensed Daycare Facilities and Onsite Wastewater installations throughout Callaway County. Trainings are conducted by CCHD in these areas for the licensed operators in Callaway and surrounding counties.
Callaway County Food Code: The new Callaway County Food Code was adopted 1-20-2012. Contact the CCHD Environmental Staff concerning any questions or concerns at (573)- 642-5750 or (573)-642-6881 ext:224 Food handler classes are held the last Tuesday of each month (with the exception of July and December). Classes are held at 4950 County Road 304 Fulton MO in the meeting room at 10 am, 2 pm and 7 pm. Classes may be scheduled at different times and locations upon request and availability of personnel.
Complaints are received in writing only. Forms are available per request.
Private water well testing for individual homes may be conducted by CCHD staff at a rate of $25.00 per test.
Onsite Wastewater Permits: Permits for the construction of new onsite wastewater systems or major modifications of an existing onsite wastewater system must be obtained from CCHD.
Sewer Permits are $200.00 and are issued regardless of the size of the property. (no acreage exclusions)
All construction of new onsite wastewater systems or major modifications of existing onsite wastewater systems must be done by a DHSS Certified Installer. Lists of these individuals are available upon request or at the DHSS website; www.dhss.mo.gov
Address Requests: Address requests are initiated with the issuance of onsite wastewater permits. To obtain a new address, a sewer permit must be obtained from CCHD, properly filled out and returned with the permit fee to the CCHD Environmental Office. Once received, a request will be forwarded from the Environmental Health Specialist to 911 Addressing, issuing a formal request for an address.
All services are provided in cooperation with the Missouri Department of Health and Senior Services. All services are performed on a non-discriminatory basis.
